Alma Palette Initial Program and Tool Implementation

Alma Palette is wound prevention program that utilizes a wound prevention tool that guides the user to identify if resident/patient skin discoloration requires attention for further assessment by a nurse or physician. Skin discoloration may be a sign of an impending skin breakdown that would require nursing intervention and or management.  The tool is recommended to be used during showers, skin sweeps, daily and weekly skin checks, incontinence care, dressing, and other ADL care where there is an opportunity to assess and observe skin for any skin alterations/impairments. Nursing staff will be warned, guided, and informed that any skin discoloration that resembles the pattern on the Alma Palette may need nurse/physician attention. This program and tool is not used for diagnosing any skin condition. It merely guides the user to notify nurse and or physician.
